 The auto-immune diseases are a diverse group of medical conditions affecting many different tissues and organs throughout the body. 
They are loosely divided into two types: organ specific and multi-organ types.

The most common disorders in primary medical practice are the Collagen vascular diseases or the Connective Tissue diseases.  Rheumatoid arthritis and its variants are perhaps the most common of these auto-immune disorders in current medical practice. They often have arthritis as a symptom.

 

There is a question as to whether there is a true distinction between organ specific and multi-organ immune diseases.  Alternately, does this simply reflect that the affected “single” tissues in multi-organ immune disease are just present in multiple organs, so their clinical effects are seen, in multiple organs? Current trends in medical practice are drifting towards the increased use of immunosuppressive medications such as Methotrexate.  This is a cell toxic immune suppressive medication.  By suppressing the immune system, inflammation is reduced and the patient feels better.  These medications are very potent in suppressing cell growth.  Some patients even on small doses may have tissue healing impaired to such an extent that they have difficulty in healing even small superficial scratches.

The treatment of these disorders is a difficult area to understand the long term consequences of clinical decisions because of the very long lag times between action and effect, clinical variability amongst patients and the complex clinical conditions of the patients involved.  (They are often elderly and have multiple medical problems). Dr. Xxxxx observes that many patients who are started on immunosuppressive medication actually respond very well initially clinically.  Inflammation is reduced and pain levels drop.  Very often in 1-2 years, the clinical condition worsens, and the patient needs to have their doses of immunosuppressive medications increased.  This usually works well and the condition again comes under control with relief for the patient.  This cycle of doses increases is played out over decades, with quite significant weekly doses of immunosuppressives being used eventually in most patients.

 

Where immunosuppressives are not used, the patients do not seem to undergo the same “clinical worsening.”

The
Paill Spectrum model predicts that Paill Spectrum has a role to play
in at least some auto-immune diseases. 

"Noel Fragment" disease as proposed by the Paill Spectrum model would suggest a new novel mechanism for the genesis of a variety of these conditions.

This site discusses a new theory (Paill Spectrum) to explain autoimmune disease in conditions such as: Chronic fatigue, Fibromyalgia, Arthritis, Sore Hands, Rheumatoid Arthritis, Connective Tissue Disease and Lupus, (SLE). There is a Paradox of methotrexate or steroid therapy in chronic treatment of these illnesses. The Paill Spectrum model suggest some new alternative approaches to therapy, for patients with worsening disease. Mechanisms of illness proposed include Noel (Noelle) Fragment Disease as well as a possible contribution from hapten iso-immunisation phenomena.
