Why is My Hair Falling Out?

In Dr. Xxxxx’s practice, this is a not infrequent problem. Many patients request a referral to a specialist such as an endocrinologist. Generally, there is no reason given for why the patient’s hair is coming out in clumps on their comb. Occasionally the patient, (especially if a young attractive female) will be diagnosed as having androgen sensitivity and be treated with a medication such as spironolactone. Occasionally, the patient even improves a bit on this medication.

In Dr. Xxxxx’s opinion, hair loss is not an issue for a knee jerk response. History and examination are very important as in most medical conditions. Back To Hair Loss Top

Diagnosis of Common Causes of Hair Loss

Male pattern hair loss is very rare in women and follows the usual “monk” like pattern of loss. i.e. It does not start all over but in specific areas.

Women who have “male pattern” hair loss need to be diagnosed with specific conditions, the most common of which is PCOS: Polycystic Ovarian Syndrome. Interestingly, these patients often show some insulin resistance, according to recent research. PCOS should not be diagnosed without other clinical or test based criteria of the disease, notably:
- Acne
- Hirsutism
- Cysts on the Ovaries

Abnormal Menses with abnormal hormonal rhythms and
Increased female version androgens such as DHEA or DHEAS. (Blood Tests Needed).
Ratios of FSH or LH to female sex steroid levels may also need to be done to ascertain the level of androgenisation secondary to changes in hormonal metabolism in non-ovulating PCOS women. Back To Hair Loss Top

Nutritional Issues are important.
- Zinc Deficiency is one likely accompaniment of hair loss, but in Dr. Xxxxx’s experience although low to normal zinc levels are common, treatment rarely results in any improvement in scalp hair levels, that is noticed by the patient or even their doctor.
- The explanation is probably that zinc replacement is a very slow process. This is often seen in the slow rise of blood zinc levels in patients taking zinc supplements, (when people are regularly blood tested). There might be long-term hair growth with zinc replacement therapy, but it occurs so slowly, that people do not really notice the change. Any changes that do occur are often attributed to other things that people have done or have taken over the preceding months, not to the zinc supplements they may have taken. Back To Hair Loss Top

The Paill Spectrum Disease Model as a Cause of Hair Loss

Paill Spectrum if it causes hair loss does so in a general “all-over” pattern of loss. The patient will not go bald. The hair loss tends to be present over months, which is consistent with the usual time scale of Paill Spectrum illness, as given by the Paill Spectrum Disease model pioneered by Dr. Xxxxx. The hair loss can be summarised as occurring in an illness pattern.

If a disease reversal can be triggered, the hair regrowth will happen gradually back to more normal levels, but again any hair growth is diffuse and is very difficult for people to notice. It is far more likely that the patient just ceases to notice or find that there is hair on their comb, than to notice any change in the amount of hair they have.

Any patient with hair loss due to Paill Spectrum will have the typical symptoms of the illness, characteristic signs of the illness and characteristic blood test changes that are characteristic of the illness. They will also respond to Paill Spectrum treatments in typical fashion. One would wonder wether those electric head zapping machines used to encourage hair growth, work by some type of heat mediated PaillSpectrum effect. :-/
